Abstract: Dementia with Lewy bodies is one of the most common causes of dementia; it is considered the second most common cause of neurodegenerative dementia in people over 75 years of age after Alzheimer's disease. Lewy bodies are formed by an abnormal accumulation of proteins in the cytoplasm, which is toxic to the neuron. In this pathology, they are mainly located in the cortex of the frontal and anterior temporal lobe, anterior cingulate, insula and substantia nigra. The most characteristic symptoms are cognitive fluctuations, visual hallucinations, parkinsonism, REM sleep behavior disorder, hypersensitivity to neuroleptics, some autonomic dysfunctions, behavioral and emotional symptoms, such as anxiety and depression, and cognitive symptoms, among which attentional, executive, visuospatial and visuoconstructive alterations stand out, with a minor affectation of memory. This paper presents the case of I.T., a 77-year-old woman with Lewy body dementia with 2.5 years of evolution. She is referred by the neurology service for the establishment of her neuropsychological profile to support the diagnosis, which is always clinical and can only be confirmed by postmortem pathological anatomy by confirming the presence of abundant Lewy bodies in the neurons of the cerebral cortex. A complete neuropsychological evaluation is performed and a comprehensive 30-session intervention plan is proposed with the aim of reducing the impact of the disease on the quality of life of the patient and her relatives.
