Abstract: Interoperability is defined as the ability of two or more systems to exchange data and information and to be able to use them subsequently. International standards have been developed to encode and model the transmission of medical data. OpenEHR proposes an open and flexible approach, based on two-level modeling and centered around the concept of the archetype: the unit with minimal and universal constraints on which to build clinical knowledge representations. The main objective of this review was to determine the current status of OpenEHR-based applications and to test their impact on the interoperability of digital health systems. Following the PRISMA methodology, Pubmed, Springer and Google Scholar databases were analyzed for the period 2018-2023 using combinations of the key concepts OpenEHR, Clinical Model and Archetype. A total of 97 articles were found, of which after filtering based on inclusion and exclusion criteria, 14 ended up being included in the final review. The range of OpenEHR applications includes Electronic Health Record Screening, epidemiology, genetics or obstetrics, among others. The community should continue to work on continuously improving the content of the OpenEHR standard, creating new archetypes and adjusting existing ones to ensure their flexibility, universality and reusability. It is important to focus on the balance between the exhaustivity of the information and the granularity of the semantics of the model.
