Abstract: The objective of this review is to compare the efficacy of low-carbohydrate-high-fat diets versus low-fat diets in body weight loss. Overweight, obesity and its associated complications are a major public health problem worldwide. The relationship between insulin resistance and obesity is indisputable, although the directionality is unknown. Numerous studies relate the glycemic index and glycemic load to obesity, through the carbohydrate-insulin model. However, important exceptions question that model. A diet rich in carbohydrates does not inevitably lead to the development of obesity. In the nutritional approach of the obese patient, there is a wide variety of dietary therapeutic proposals, among which are low-carbohydrate-high-fat (LCHF) diets and low-fat 4 / (LF) diets. Both types of diet have shown the same efficacy in weight loss, but their long-term adverse effects are unknown, especially at the cardiovascular level. According to current scientific evidence, the efficacy of one or another diet in weight loss cannot be related to the genetic profile of the individual or the pattern of insulin secretion. A diet low in carbohydrates improves the lipid profile of obese patients more effectively than a diet low in fat, so it could be considered supervised adoption in patients with cardiometabolic disorders.
