Title: Sensibilidad al gluten no celíaca: revisión bibliográfica

Abstract: In recent years, a new syndrome called non-celiac gluten sensitivity (NCGS) has gained interest and wide visibility both in the scientific community and the general population. NCGS is included in gluten-related disorders and is characterized by intestinal and extra-intestinal symptoms related to the intake of foods containing gluten, in subjects without celiac disease or wheat allergy. To date, many unresolved questions remain regarding the clinical manifestations, diagnosis, or treatment of this syndrome. The aim of this bibliographic review was to collect relevant information to clarify controversies on this topic The bibliographic search consisted of consulting the PubMed and CINAHL Complete databases during November 2020, using the term: "Non-celiac gluten sensitivity". Finally 62 studies (46 reviews and 16 clinical trials) were included. NCGS remains an exclusion diagnosis for celiac disease and wheat allergy. There are no specific reliable biomarkers or histological abnormalities that can confirm the diagnosis, and the only way to diagnose these patients is by the self-reported response to a gluten-free diet or by performing a double-blind gluten-placebo provocation test. A gluten-free diet is the most effective treatment for patients with NCGS. Self-diagnosis should be avoided and it is recommended to follow this diet under professional supervision to reduce the risk of nutritional deficiencies.
