Title: La intervención nutricional como herramienta para reducir la fragilidad de los pacientes con Enfermedad Renal Crónica Avanzada (ERCA) en lista de espera de trasplante renal fallecido

Abstract: Frailty is a physiological status that causes patients to become more vulnerable when they face/ have a higher risk of developing complications when they face a stressful event such as kidney transplant. Research has been performed on frailty and it is associated with an increased risk of adverse outcomes, including falls, hospitalization, high mortality rate and poor graft survival after transplant. Prehabilitation intervention or multimodal conditioning in the three spheres of the patient during the perioperative period (physical, nutritional and emotional health) may reduce frailty, so it could improve results after transplant. We carried out a research study on nutritional intervention (it is included in a multimodal intervention by multidisciplinary healthcare sectors) to assess program effectiveness in patients on the kidney transplant waiting list. Patients included are Stage 5 Chronic Kidney Disease (CKD) patients at the Hospital del Mar. Thereafter, we screened patient nutritional status according to GLIM criteria for the diagnosis of malnutrition in order to predict the risk of frailty. The population sample used for this preliminary analysis takes part in a randomized clinical trial that is still in the open recruitment (Frailmar) and it is small. However, preliminary results are encouraging and we expect this trend to continue when the sample increases. Further studies are needed in order to evaluate the benefits of prehabilitation with an adequate nutritional intervention. Prehabilitation should soon become part of the standard care in the most important Transplantation Units of our country if benefits are proved.
