Title: Efectos de los antituberculostáticos de primera línea sobre la microbiota intestinal

Abstract: Objective The use of anti-tuberculosis drugs (anti-TB) can alter the intestinal microbiota of the host. A literature review of published research on the effects of firstline anti-TB treatment on the gut microbiota is carried out. Methods MEDLINE, EMBASE and AMED were searched for articles published between 2010 and 2020, including all study designs. Preferred reporting elements for systematic reviews, meta-analyzes and standard Cochrane methods were followed. Narrative synthesis was used to inform around the themes that emerged from the data. Results Fifteen articles showed that anti-TB drugs have an impact on the intestinal microbiota, causing a decrease in bacterial diversity and relative changes in the proportions, effects that can be different, long-lasting and facilitate relapses and reinfections. Anti-TB-induced disruption of the microbiome can have significant effects on peripheral immune responses and overall systemic immune tone. The most important effects on the gut microbiota are mainly attributable to rifampicin. Although the alterations can be reversible, the recovery time can be long and incomplete. The implementation of probiotics can improve the efficacy of anti-TB drugs to treat the disease and improve host immunity. Conclusions There is evidence that first-line anti-TB drugs used for TB treatment impact the composition and diversity of the intestinal microbiota. Larger population studies are needed to fully understand how anti-TB drugs modulate the microbiota and to determine their impact on general health.
