Title: Recerca bibliogràfica dels factors socials determinants en el procés de transició entre sistemes de salut mental ambulatòria: Contribucions del treball social Sanitari en el pas efectiu del centre de salut mental infanto-juvenil al centre de salut mental d'adults

Abstract: There are several social factors that affect the different stages through which crosses the mental illness; one of them is related to the change in the center of adult outpatient mental health, where the teenager hit a severe mental disorder should be adapted to the new dynamics of attention and team of professionals assuming it multiple times a stressful situation that can affect the correct linking and tracking back to new device. In the present work a bibliographic research on the main factors that determine this transition between outpatient mental health devices and which are likely to be seized from the Medical Social Work to promote the optimum grip and to contribute to the continuity of care of the adult mental health center. The approach of the social forces in the exercise of understanding health and illness from an integrating approach where the three types of factors (biological, psychological and social) developed a role just as significant in the mental health of young people with severe psychiatric pathology in a time of change as part of this research.
