Abstract: Sarcopenia is defined as the loss of muscle mass, function and strength, emerging as an important prognostic factor in several types of cancer. It has been associated with aging, malnutrition and toxicities secondary to oncological treatment. It is a prevalent condition in cancer patients regardless of the nutritional status and stage of the disease, which is associated with increased mortality and poor control of the disease. The molecular mechanisms related to sarcopenia are inflammation, proteolysis, protein synthesis deficit and mitochondrial oxidation. When omega-3 fatty acids are incorporated into cell membranes, they modify transduction, prostaglandin synthesis and produces pro-resolving mediators, promoting anti-inflammatory action and inhibition of the catabolic pathways. The beta-hydroxy-beta-methylbutyrate (HMB) has been shown to affect muscle protein turnover by stimulating protein synthesis via up-regulation of anabolic signaling pathways and by decreasing proteolysis via down-regulation of catabolic signaling pathways. The exercise has an anti-inflammatory effect, that is shown to be effective at counteracting the muscle catabolism by increasing protein synthesis and reducing protein degradation, thus successfully improving muscle strength, physical function and quality of life in patients with cancer. Nutrition and exercise are potentially applicable measures for the prevention and treatment of sarcopenia related to cancer, which should be initiated at diagnosis and maintained during and after treatment.
