Title: Aproximación a la aplicación del modelo de atención a la complejidad desde el enfoque del trabajo social sanitario

Abstract: The growth of life expectancy has generated the phenomenon of demographic aging, consequently increasing older people with chronic and complex diseases and at the same time with more social problems. The purpose of this study is to explore and describe the reality of the complexity care model in Complex Chronic Patients and patients in need of an Advanced Chronicity Attention Model admitted to the Hospital Dos de Maig (HDM), through a cross-sectional descriptive study and using the quantitative methodology. The sample focuses on the complex chronic patient and advanced chronicity patient discharged in hospitalization units. The results suggest that the theory of the attention to complexity model is not feasible. Most of the patients analyzed did not have an annual review of the social assessments from primary care or interventions from the Hospital Dos de Maig, they had neither the advance decision planning nor the advance directives document and the individualized and shared intervention plan did not have socio-familiar information. This work concludes that the increase of people with complex chronic diseases, the existing human resources of social health work and the time of hospital stay limit the interventions of social health work from primary and hospital care. On the other hand, it is indicated the reduced participation of the social health workers in the elaboration of the individualized and shared intervention plan and the lack of internalization of the bases of the person centered care.
