Title: La atención integral hospitalaria. De trabajadora social a trabajadora social y sanitario

Abstract: The demographic changes in recent years have generated a change in epidemic patterns, with the prevailing pluripathology currently. That leads to an increase in dependency, which means difficulties in the daily environment. People who need social care and their families expect to receive a simultaneous, coordinated and stable response that meets their needs. The social workers are key in the integral valuation, where the coordination with the social services of the community is fundamental to guarantee the continuity of the attention to the hospital discharge, taking into account the project of life of each person. This study aims to determine, using Osakidetza as a reference and more specifically the University Hospital of Donostia, the importance of the figure of the social health worker in the hospital's interdisciplinary team to provide a person-centered care that guarantees continuity of care when it returns to the community.
