Abstract: An association between high plasma cholesterol levels, use of statins and lower risk of Parkinson's disease (PD) has been showed. However, it is unknown if the clinical picture differs among de novo drug naïve PD patients with and without hypercholesterolemia. The aim of this study was to evaluate clinical differences among PD patients with and without hypercholesterolemia, the effect of statins treatment and the correlation between clinical variables and cholesterol plasma levels. Three hundred and seventy five patients were evaluated of whom 97 had hypercholesterolemia. They underwent a motor and neuropsychological evaluation including a psychiatric exam. The frequency of statins intake was calculated. There were no significant differences between both groups of patients in any of the studied variables. Most patients with hypercholesterolemia were taking statins, therefore its role in the clinical picture could not be assessed. Higher levels of total and LDL cholesterol correlated with lower scores in memory tests and a higher number of depressive symptoms. Hypercholesterolemia does not entail a different clinical picture in the novo PD patients. However, higher plasma total and LDL levels correlate with worse memory performance and a higher number of depressive symptoms, supporting a negative role of hypercholesterolemia in PD. Given the high frequency of statins intake its role in PD symptoms could not be assessed.
