Title: La soledad y el aislamiento en la gente mayor. El papel del TSS en atención primaria

Abstract: Unwished loneliness and isolation in the elderly is a growing problem in most societies. This work aims to show how Health Social Workers (HSW) in primary care are key professionals to identify elderly people with perception of unwished loneliness and isolation. To define how loneliness and isolation have a negative impact on the elderly¿s health, how it can be curbed through an active and friendly aging, and the interventions and detection by HSWs. This work takes place in Baix Llobregat Litoral and Hospitalet de Llobregat Sud areas, where I work as a health social worker. I have performed 15 interviews to HSWs from primary care in the area to know how loneliness and isolation in people older than 65 is addressed, identified, prevented and treated from their point of view. HSWs are key to primary care and they work in coordination with the rest of health care providers. Through their interventions and social diagnosis, they are able to address unwished loneliness and isolation of the elderly, which improves their health and minimizes the negative impact that unwished loneliness can have. Therefore, their help embraces physical, social and psychological areas. HSW in primary care are key to identify the elderly people with a perception of unwished loneliness. By working in a coordinated and/or proactive way with the rest of health care providers and/or teams, vulnerable people are identified and loneliness and isolation in the elderly can be addressed by promoting a healthy and active aging.
