Abstract: Phenylketonuria (PKU) is an inborn error of metabolism caused by a deficiency in the activity of the liver enzyme phenylalanine hydroxylase. This enzyme metabolises phenylalanine (Phe) to tyrosine (Tyr). The diagnosis is usually made by neonatal screening within the first 72 hours of life. The enzyme deficiency leads to accumulation of phenylalanine and, if left untreated, can cause irreversible neurological damage. Despite the new treatment approaches, the most suitable treatment keeps begin dietary patterns. Those patterns include following a very controlled diet of protein content during the patient’s lifetime. Tolerance to protein intake can vary between individuals and relies on enzyme activity, being ≤ 10 g protein/day for most PKU individuals. Therefore, natural protein is replaced by protein substitutes which are nutritional formulas containing a mixture of amino acids, with low or no phenylalanine content, supplemented with tyrosine, micro and macronutrients. The nutritional composition of protein substitutes and the amino acid or peptide content influences digestion, absorption kinetics and subsequent availability. During strength training, protein substitutes can be used for recovery intake. Given that there are no specific recommendations for adult’s PKU. The aim of this dissertation is to provide specific recommendations on the type of formula, amount and timing of the protein intake in order to improve the efficacy and safety of the use of protein substitutes and therefore maintaining a good metabolic control.
