Abstract: In today's society, the importance of mental health has increased, leading to an enhanced focus on early detection of mental disorders and placing more emphasis on individual functionality rather than the symptoms themselves. One paradigmatic case is the significance of academic attainment in adolescents with mental disorders or distress. Likewise, the importance of a gender perspective in mental health has been increasingly recognized. In this study, we concentrate on adolescents exhibiting symptoms of the schizophrenia spectrum from a gender perspective. Delving into this aspect could help us ensure better and fairer academic attainment, intervening earlier and more specifically when necessary. Therefore, this work opts for a narrative review, following the PRISMA method to enhance its quality, aiming to synthesize and analyze the available evidence regarding gender differences in academic achievement among adolescents with symptoms of schizophrenia. Through the analysis of 9 studies, relevant information is obtained regarding gender differences in clinical presentation and their potential or observed impact on academic attainment. It is observed that girls exhibit more negative symptoms, while boys have more severe symptoms, greater cognitive deficits, and social problems. Finally, it is concluded that girls tend to have higher academic achievement than boys, which is hypothesized to be related to gender differences in clinical presentation and/or the natural course of schizophrenia spectrum disorders.
