Abstract: The worldwide average prevalence of ASD is 1 out of 100 children. Although the proportion of females has been established at 1:4 with respect to males in the clinical population, different studies point out that this proportion would be incorrect, since there is underdiagnosis of female ASD, especially in those who have their cognitive abilities preserved. Searches were carried out in the PsycInfo, Psychology Database and PubMed databases for articles published between 2013 and 2023. Of the 103 studies found, 22 met the inclusion critera, analyzing in some aspect the relationship between the female gender and ASD. Amongst the results, no gender difference has been identified in babies with ASD. Instead, girls from preschool age on, tend to internalize behaviors, have more social skills and perform more camouflage strategies, which places them in a situation of vulnerability in screenings. It has been found that those tools that have been evaluated for the diagnosis of ASD are biased towards the male gender. The female underdiagnosis of ASD and the need to focus on early childhood intervention, where gender differences are less obvious and girls with less severe symptoms go more unnoticed.
