Avaluació i intervenció psicològica en un cas d'Alzheimer

Abstract

Examen d'un cas real de la malaltia d'Alzheimer amb altres patologies. Amb aquest treball es presenta a un pacient amb la malaltia d’Alzheimer diagnosticada prèviament que acudeix a consulta per presentar dificultats de memòria i comunicació. La malaltia d’Alzheimer es considerada “un procés neurodegeneratiu del sistema nerviós central caracteritzat per una mort neuronal progressiva en certes zones del cervell, concretament l’escorça cerebral i l’hipocamp són dues de les zones més afectades per aquesta malaltia” (Martínez, A. 2009). Els resultats trobats en el protocol realitzat al pacient d’avaluació neuropsicològica, mostren alteracions en la memòria tant immediata com demorada, amb un rendiment baix en funcions executives i amb graus problemes amb el llenguatge. La resta d’àrees es troben bastant preservades. Tot aquestes dades, conjuntament amb les extretes de la entrevista familiar i de la seva valoració clínica global de la malaltia esmentada, recolzen la diagnosi de malaltia d'Alzheimer. El pacient necessita de recolzament en algunes de les activitats bàsiques de la vida diària i el seu estat d’ànims, és satisfactori. L’objectiu general de la intervenció ha estat preservar el funcionament cognitiu del pacient de les funcions cognitives menys afectades, per mirar d’atorgar-li una major autonomia. Els objectius específics han estat centrats en la millora de les habilitats comunicatives del pacient, per així poder millora les relacions amb el seu entorn. La intervenció proposada en aquest treball s’ha centrat en una proposta multidisciplinar individualitzada d’entrenament neurocognitiu, recolzada per activitats mitjançant un software de rehabilitació cognitiva i treball en format paper, complementat per activitat física regular i millora de les interaccions socials.

In this work I present a patient with previously diagnosed Alzheimer’s disease. He comes to consultation to present memory and communication difficulties. Alzheimer’s disease is considered “a neurodegenerative process of the central nervous system characterized by progressive neuronal death in certain areas of the brain, specifically the cerebral cortex and the hippocampus are two of the areas most affected by this disease.(Martínez, A. 2009). The results found in the patient’s neuropsychological assessment protocol, show alterations in both immediate and delayed memory, with poor performance in executive functions and with degrees of language problems. The rest of the areas are quite preserved. All these data, together with the extracts from the family interview and from their overall clinical assessment of the aforementioned disease, support the diagnosis of Alzheimer disease. The patient needs support in some of the basic activities of daily life and his mood is satisfactory. The general objective of the intervention has been to preserve the patient's cognitive functioning of the least affected cognitive functions, in order to try to grant him greater autonomy. The specific objectives have been focused on improving the patient's communication skills, so that they can improve their relationships with their environment. The intervention proposed in this work has focused on an individualized multidisciplinary proposal of neurocognitive training, supported by activities through cognitive rehabilitation software and work in paper format, complemented by regular physical activity and improvement of social interactions.
