Més enllà del diagnòstic: on són les nenes amb TEA? Eficàcia del tractament i les intervencions del TEA d’acord a la simptomatologia diferenciada associada al gènere

Abstract

Introducció. En la darrera dècada hi ha hagut un increment en el diagnòstic del TEA, especialment en les dones, degut a una revisió exhaustiva de les característiques inherents al gènere que mostren una presentació diferenciada de símptomes i grau de funcionalitat. Malgrat aquests avenços, encara existeix una gran escassetat en la literatura de propostes de tractaments diferenciats per gènere. Objectius: Recollir informació sobre la efectivitat d’aquest tipus d’intervencions i tractaments incidint en la comparativa dels resultats per génere. Mètode: Es porta a terme una revisió sistemàtica seguint els protocols PRISMA emprant les fonts d’informació de Pubmed, Scopus i PsycINFO, excloent per edat >18, manca de diferenciació per sexe en els resultats i tipus de teràpia. Resultats: Se seleccionen i analitzen un total de 10 estudis, dels quals 7 mostren resultats diferenciats per gènere, indicant que les intervencions centrades en regulació de la conducta i integració sensorial són menys efectives en noies, mentre que els tractaments psicoeducatius i aquells enfocats als símptomes internalitzants són més efectius en aquestes. Conclusions. Es discuteixen els resultats d’acord a la literatura actual i es proposen orientacions per a futurs estudis centrats en la rellevància de la perspectiva de gènere en l’adequació i personalització d’aquests tractaments.

Background. In the last decade there’s been an increase in the diagnosis of ASD, especially in women, because there’s been an exhaustive review of the inherent characteristics related to gender, which show that there is a differentiated exhibition of symptoms and functionality. Despite these advances, there is still few evidence in the literature about the proposal and analysis of differentiated treatments according to those characteristics. Objectives. To report information about the effectiveness of this type of interventions focusing on the differences in the results related to gender. Method.We carry out a systematic review following PRISMA guidelines including data source from Pubmed, Scopus and PsycINFO, excluding for population age >18, lack of gender differentiation in the results and type of treatment. Results: 10 studies are classified and analysed, finding differences in efficacy according to gender in 7 of them, which indicate that behavioural and sensory integration are more effective in boys, while psychoeducation and treatments focused on internalizing symptoms are better in girls. Conclusions. Results are discussed regarding the current literature, and we propose some orientations for future studies focusing on the importance of the gender perspective in the suitability and personalization of these treatments.
