Title: Análisis de salud y comorbilidad a partir de los diagnósticos de salud en la historia clínica informatizada en un centro de Atención Primaria de Reus

Abstract: In the 2016-2020 health plan of the Generalitat of Catalonia, a new model of primary health and community care is proposed, and community health is becoming more important. To start working on the community health of a population, the first step is to make a health diagnosis. This study focuses on the realization of the quantitative part of this health diagnosis where an observational and cross-sectional study of the population attended in our health center is carried out. As a result, we have obtained that in the age range between 35 and 54 years is where there is a greater population and it is the male sex is the most represented. We obtain differences between the sexes in the adjusted morbidity group score, the aging index and the average number of diagnoses. There are also differences between sex and age groups with calculated comorbidity rates, Charlson's and Elixhauser's. And the Elixhauser index is the one that has the highest correlation with the adjusted morbidity group. And there are differences between the prevalences in our population and the national ones in the diagnoses of dyslipidemia, arterial hypertension, chronic renal failure and chronic obstructive pulmonary disease. In conclusion, the female population over 64 has a higher comorbidity index than the rest of the population and that there are some diagnoses with lower prevalence than at the national level.
