Abstract: This Final Degree Project presents a new design of the application "My Health" created by the Generalitat de Catalunya in 2015 which contains the health documentation of Catalan citizens as well as offering different services and information that allow online consultation. The current health situation makes it necessary that people can access their personal health information to consult tests, diagnoses, medication, among others and also perform remote management so that they avoid having to travel in person to the primary care center and thus help to unclog the public health. To carry out this project, the User-Centered Design (UCD) methodology is applied, which allows the analysis of the current state of the "My Health" application and user analysis to detect and understand their problems and needs and then carry out the design of user profiles and information architecture with the intention of creating different redesign proposals or prototypes that will be subsequently evaluated in terms of usability by real users, always placing them at the center of the process, involving them in the different phases. This whole process results in the creation of a final interactive application that guarantees an improved user experience regardless of the user's capabilities and the technological device to be used.
