Abstract: Breastfeeding is the ideal food for optimal growth and development of children and an essential tool for reducing infant mortality and morbidity. The composition of milk is dynamic, adapting to the needs of the infant at each moment, and one of the determinants of its composition is the maternal diet. The aim of this work is to describe the effect of maternal diet on macronutrients, micronutrients and microbiota of breast milk. A literature review of the last five years, from 2017 included, has been carried out. The identification and selection of the 28 studies included to the review has been carried out according to the Preferred Reporting Items for Systematic reviews and Meta-Analyses (PRISMA) statement. In relation to milk macronutrients, diet has a greater influence on the fatty acid profile than on milk carbohydrates or proteins. As for milk micronutrients, there is a relationship between the maternal diet and the milk content of vitamins, but not of minerals. Finally, breast milk provides a microbial source for infant intestinal colonization and the maternal diet influences the composition and diversity of the milk microbiota. This information will have to be taken into account in future dietary interventions with mothers to maximize the benefits of breastfeeding and the influence on the health of infants, especially on their growth, development and maturation of the nervous system and immune function.
