Pràctica clínica dels trastorns dels sons de la parla (TSP) d'etiologia funcional

Abstract

Estudis recents aporten evidències de l’existència d’un retard en el processament neurofisiològic de la informació fonològica en els infants amb trastorns dels sons de la parla (TSP) d’origen funcional. La present guia de pràctica clínica (GPC) analitza el processament de la parla i preveu que les errades fonètico-fonològiques poden estar relacionades amb un dèficit o ruptura en un o més aspectes de la cadena de processament de la parla. S’ha considerat que realitzar una cerca basada en l’evidència científica sobre el processament de la parla com a possible etiologia dels trastorns funcionals pot influir positivament en l’aplicació d’intervencions fonètico-fonològiques eficients i efectives. Per contrastar la hipòtesi plantejada, s’ha dut a terme una cerca bibliogràfica a les bases de dades PubMed, Semantic Scholar i ASHA on s’han obtingut 242 articles dels quals 10 han estat seleccionats seguint el procediment proposat per PRISMA. La qualitat metodològica i de report dels estudis seleccionats ha estat avaluada seguint la metodologia suggerida per Scottish Intercollegiate Guidelines Network (SIGN). S’han formulat 14 preguntes clíniques adreçades al processament de la parla, el desenvolupament fonològic, enfocaments d’avaluació i enfocaments d’intervenció per, seguidament, concretar les recomanacions. L’evidència ha reportat que un dèficit en el processament podria justificar la presència d’un TSP funcional, però seria recomanable plantejar noves investigacions que profunditzin en els processos subjacents. S’ha dissenyat un protocol per l’atenció logopèdica dels TSP funcionals amb la finalitat d’aportar eines als professionals que facilitin la presa de decisions i el raonament clínic respecte a l’avaluació i la intervenció.

Recent studies provide evidence of the existence of a delay in the neurophysiological processing of phonological information in children with speech sound disorders (SST) of functional origin. This clinical practice guideline analyzes speech processing and predicts that phonetic-phonological errors may be related to a deficit or break in one or more aspects of the speech processing chain. It has been considered that conducting a search based on scientific evidence on speech processing as a possible etiology of functional disorders can positively influence the application of efficient and effective phonetic-phonological interventions. To test the proposed hypothesis, a bibliographic search was carried out in the PubMed, Semantic Scholar and ASHA databases where 242 articles were obtained, 10 of which were selected following the procedure proposed by PRISMA. The methodological and reporting quality of the selected studies has been evaluated following the methodology suggested by the Scottish Intercollegiate Guidelines Network (SIGN). Fourteen clinical questions aimed at speech processing, phonological development, assessment approaches and intervention approaches have been formulated to then specify the recommendations. Evidence has reported that a processing deficit may account for the presence of a functional TSP, but further research is needed to delve into the underlying processes. A protocol has been designed for the speech therapy care of children with functional TSP to provide professionals with tools that facilitate decision-making and clinical reasoning regarding assessment and intervention.
