Title: Programa de planificación del alta dirigido a pacientes mayores de 65 años y familiares con Ictus de un centro residencial de media o larga estancia

Abstract: Stroke is located worldwide, European and Spanish, among the leading causes of death, constituting itself a problem of the first order. It is expected that the problem will increase in Spain, given the progressive aging of the population, and as age is one of the factors that facilitate the onset of the disease. Stroke occurs unexpectedly in the patient, producing a series of sequels at a physical, psychological and social level, which will affect their lives, and may affect the carrying out of activities of daily life and the organization of the family dynamics. The hospital discharge and the referral to a rehabilitation center indicated by medical prescription will allow a better recovery of the patient as a step prior to his return to the family home. For this, it is necessary that, from this rehabilitation resource, work is carried out with the patient and his family in order to detect the presence of social needs derived from the stroke and to launch an intervention plan in order to address them, and thus, prepare the discharge of the residential rehabilitation center in order to achieve an improvement in the functional and cognitive capacities that facilitate their return to the patient's home environment. All this will be done under the umbrella of interdisciplinary coordination and always with the participation of the patient and his family in the preparation of the Discharge Plan.
