Abstract: Obesity increases worldwide and it is a big public health problem. Central obesity is associated to metabolic disorders such as hypertriglyceridaemia, dyslipidaemia, hypertension and insulin resistance. This dysfunctions set is known as Metabolic Syndrome (MS). The physiopathological mechanism is not well known but the progress in the adipose tissue knowledge has shown that in obesity conditions, adipocytes produce a high proinflammatory cytokines secretion and macrophages infiltration, with systemic and tissues afections, what is known as chronic low grade inflammation. In this way a potential relationship between obesity, inflammatory biomarkers and MS is established. Changes in food and lifestyle are considered fight key factors against these diseases. Dietary inflammatory index (DII) is shown as a tool to measure potential inflammatory diet and to assess dietary patterns appropriate for its prevention and nutritional treatment. This work is a bibliographic research study related to these premises in order to draw conclusions. The results found confirm that there are different biomarkers inflammatory activity related to SM and obesity.They also hold a relationship between high DII (proinflammatory) and obesity, which is not so obvious to the MS. Mediterranean dietary pattern has been choiced as the best nutritional intervention. Technical and design limitations current studies make their results inconclusive. The investigation future must be leaded to clinical trials, statistically significant, applying "omic" sciencies and using the available technological innovations.
