Abstract: Patient presentation: this case is a 44-year-old woman in a minimally conscious state (CME) following severe traumatic brain injury in the context of a traffic accident 5 months ago. Pathology description: CME is a severe alteration of the level of consciousness in which there is minimal presence, but clearly perceptible evidence of awareness of oneself or the environment. At the family level, this situation is a traumatic event, and the care work is an intense and long-term stressor that generates psychological anguish. Assessment results: the patient shows severe alteration of attentional processes and communicative ability; possible moderate-severe impairment of memory and executive functions and severe functional impairment. The evaluation of the family environment shows probable difficulties in the adaptation to the situation and slight overload of care. Intervention goals: Improve the patient's overall functioning and reduce the impact of sequelae (cognitive, emotional and behavioral) on her quality of life and that of her family. Intervention description: A twelve-month intervention program with four weekly sessions is proposed which is divided into three phases: (1) encourage family adaptation and increase the patient's level of alertness and awareness; (2) promote neurorehabilitation of higher functions; (3) facilitate adaptation and the return to their home. The intervention includes individual (patient or family) and joint (patient-family) sessions of variable duration (20-60 minutes).
